Unfortunately there's little I can do about the survey stuff. It's everywhere, and while annoying, it's not malicious. FTR, I don't get paid for those specifically.

Instead of saying the hot girls thing, I am getting the same ad but it says "Are you sure you don't want this one time offer?". In Chrome, first visit of the day.

The ad on the side was a quit smoking PSA, and the one on the top was for CIBC. Don't know if that helps you identify the culprit or not...

I don't know, but it can't hurt if we keep collecting the info. Disclaimer for them: it's most likely not the advertiser showing doing this but some ad network delivering bad code.
